DELLA NOSTRA PRESIDENTE, GIADA DA ROS, PERCH’ NON VENGA INCLUSA LA
CFS/ME NEL DSM-5

Members of the DSM-5 Task Force,
On
behalf of the CFS Associazione Italiana – CFS Italian Association of
Aviano (Pordenone, Italy), I want to join the International Association
for Chronic Fatigue Syndrome (IACFS/ME) in the content of their letter
and their deep concern about the possible reclassification of CFS as a
somatoform disorder in DSM-5, either as a stand alone psychiatric
disorder or as a subcategory under the umbrella-diagnosis of Complex
Somatic Symptom Disorder (CSSD). As pointed out by the President of the
IACFS/ME it ’would not be reasonable based upon the body of scientific
evidence and the current understanding of this disease’ – CFS/ME, as it
is understood by the medical community and explained by the related
literature is a multi-system physiological disorder – and we, too,
believe it would be counterproductive both for the sufferers and for
the physicians who try to effectively care for their patients.
Many
of our members have been treated from a psychiatric point of view and
most often than not the effects on their lives have been devastating,
with no positive results whatsoever, and this not just according to
them, but according to the professionals who were treating them. The
psychiatric treatment added insult to injury and worsened their
condition. As already clearly stated by the IACFS/ME, CFS/ME is not
clinical anxiety or depression, nor should it be classified as CSSD:
there are somatic symptoms (criterion A) and there is chronicity
(criterion C) but there are certainly not ’misattributions’ nor
’excessive concern or preoccupation with symptoms and illness’
(criterion B) in patients with CFS/ME, though in the absence of
available objective clinical tests, CFS patients may also meet
criterion B-4 (Belief in the medical seriousness of their symptoms
despite evidence to the contrary). To this regard we concur with the
CFIDS Association of America when it states that ’as drafted, the
criteria for CSSD establish a ’Catch-22’ paradox in which six months or
more of a single or multiple somatic symptoms ’ surely a distressing
situation for a previously active individual ’ is classified as a
mental disorder if the individual becomes ’excessively’ concerned about
his or her health. Without establishing what ’normal’ behavior in
response to the sustained loss of physical health and function would be
and in the absence of an objective measure of what would constitute
excessiveness, the creation of this category poses almost certain risk
to patients without providing any offsetting improvement in diagnostic
clarity or targeted treatment.’ We see the need to work (with research
and studies) to better the ability of medicine to collect the
appropriate evidence, not to make patients change their minds on the
seriousness of their symptoms.
The risk here is also to
exchange cause and effects. Patients do not have a psychiatric
condition that manifests itself in somatoform symptoms, and to make
such a statement, let alone giving it recognition within the DSM-5,
would be ruinous to a proper medical diagnosis and treatment. What is
true is that the symptoms of the disease are so intense and prolonged
that they could also cause psychological effects for which often the
patients would benefit from psychological support: debilitating
physical and cognitive symptoms, isolation due to the disease, lack of
understanding and needing to defend themselves against accusations of
malingering or of having everything ’all in their head’, being blamed
for their condition, losing the trust of people around them, having to
abandon their former lifestyles, not being able to trust in their
future’ these are true effects with which patients have to deal, a big
toll that needs to be addressed by the caregivers. If these effects are
framed as a cause, we believe that would not be possible.
We
think you should also be concerned about your own scientific integrity.
The history of the DSM regarding other situations shows that it’s not
immune to regrettable mistakes, based on lack of understanding and
knowledge. CFS/ME is a disease that has an unfortunate past of
misunderstanding and bureaucratic abuse. It is a complicated medical
puzzle that dedicated physicians are trying to solve and whose
scientific effort has been hindered by narrow-mindedness, in the past.
After decades, the wave is starting to change direction and we have
assisted to many breakthroughs and to the accumulation of clinical
evidence that simply cannot be ignored. Researchers even hypothesize
that we might have a biomaker in a year or two, if not a cure. In front
of all the evidence that researchers better than us can explain to you,
and that it is still being evaluated and studied, we believe that
making the self-assured statement that CFS/ME is a psychiatric
condition is not only arrogant, but unconscionable. You should learn
from your own past.
In conclusion, we urge you not to include
CFS/ME in the DSM-5, for it is not necessary, harmful and not
scientifically sound, on the basis of the medical knowledge that we
have.

unisco alla Associazione Internazionale per la Sindrome da Fatica
Cronica (IACFS/ME) nel contenuto della sua lettera e nella sua profonda
preoccupazione riguardo alla possibile ri-classificazione della CFS
come disturbo somatoforme nel DSM-5, sia come disturbo psichiatrico a
se stante che some subcategoria sotto la diagnosi-ombrello di Disturbo
dai Sintomi Somatici Complessi (CSSD). Come ’ stato fatto notare dal
presidente della IACFS/ME ’non sarebbe ragionevole sulla base del corpo
di prove scientifiche e della corrente comprensione di questa malattia’
’ la CFS/ME, come ’ compresa dalla comunità medica e spiegata dalla
relativa letteratura, ’ un disturbo fisiologico multisistemico ’ e pure
noi crediamo che sarebbe controproducente sia per coloro che soffrono
che per i medici che cercano di curare in modo efficace i propri
pazienti.
Molti dei nostri membri sono stati trattati da un
punto di vista psichiatrico e più spesso che no gli effetti sulle loro
vite sono stati devastati, con nessun effetto positivo di alcun tipo, e
questo non solo secondo loro, ma secondo i professionisti che li
stavano trattando. Il trattamento psichiatrico ’ stato oltre al danno
la beffa e ha peggiorato la loro condizione. Come già chiaramente
dichiarato dalla IACFS/ME, la CFS/ME non ’ ansia o depressione, n’
dovrebbe essere classificata come CSSD: ci sono sintomi somatici
(criterio A) e c’’ cronicit’ (criterio C), ma non ci sono sicuramente
’errate attribuzioni’ n’ ’eccessiva apprensione o preoccupazione sui
sintomi e la malattia’ (criterio B) nei pazienti con la CFS/ME, sebbene
in assenza di obiettivi test clinici a disposizione, i pazienti con la
CFS possano anche rispondere al criterio B-4 (Credere nella seriet’
medica dei propri sintomi nonostante la prova del contrario). A questo
proposito concorriamo con la CFIDS Association of America quando
dichiara che ’così come redatti, i criteri per il CSSD istituiscono un
paradosso alla ’Comma-22’ in cui sei mesi o più di un singolo o di
multipli sintomi somatici ’ di certo una situazione sconvolgente per un
individuo in precedenza attivo ’ sono classificati come disturbo
mentale se l’individuo diventa ’eccessivamente’ preoccupato riguardo
alla propria salute. Senza stabilire quale sarebbe un comportamento
’normale’ in risposta alla prolungata perdita di salute fisica e
funzionalit’ e in assenza di una misura oggettiva di quello che
costituirebbe eccessivit’, la creazione di questa categoria pone quasi
certamente un rischio per i pazienti senza offrire nessun compensatorio
miglioramento in chiarezza diagnostica o trattamento mirato’. Noi
vediamo la necessità di lavorare (con ricerche e studi) per migliorare
la capacità della medicina di acquisire le prove adeguate, non
per far cambiare idea al paziente sulla seriet’ dei
sintomi.
Il rischio qui ’ anche quello di
scambiare la causa con gli effetti. I pazienti non hanno una patologia
psichiatrica che si manifesta in sintomi somatoformi, e fare una simile
dichiarazione, e tanto più darvi riconoscimento all’interno del DSM-5,
sarebbe rovinoso per una appropriata diagnosi medica e trattamento.
Quello che ’ vero ’ che i sintomi della malattia sono così intensi e
prolungati che potrebbero anche causare degli effetti psicologici per
cui spesso i pazienti beneficerebbero di un supporto psicologico:
debilitanti sintomi fisici e cognitivi, isolamento dovuto alla
malattia, mancanza di comprensione e necessità di difendersi contro le
accuse di fingere o di avere tutto ’nella propria testa’, venir
colpevolizzati per la propria condizione, perdere la fiducia delle
persone intorno a s’, dover abbandonare il proprio precedente stile di
vita, non essere in grado di credere nel proprio futuro’ questi sono
veri effetti che i pazienti devono affrontare, un grande costo
psicologico che c’’ bisogno che venga affrontato da coloro che si
prendono cura degli ammalati. Se questi effetti venissero formulati
come causa, crediamo che questo non sarebbe possibile.
Pensiamo
che dovreste essere preoccupati anche riguardo alla vostra integrit’
scientifica. La storia del DSM riguardo ad altre situazioni mostra che
non ’ immune da deplorevoli errori, basati sulla mancanza di
comprensione e di conoscenza. La CFS/ME ’ una malattia che ha uno
sfortunato passato di incomprensione e abuso burocratico. ’ un puzzle
medico complicato che dedicati medici stanno cercando di risolvere e il
cui sforzo scientifico ’ stato ostacolato dalla ristrettezza di vedute,
in passato. Dopo decadi, l’onda sta cominciando a cambiare direzione e
abbiamo assistito a molte scoperte e all’accumulo di prove cliniche che
semplicemente non possono essere ignorate. I ricercatori hanno
ipotizzato che potremmo avere un marker biologico in un anno o due, se
non una cura. Di fronte a tutte le prove, che i ricercatori meglio di
noi possono spiegarvi, e che ancora vengono valutate e studiate,
crediamo che fare una dichiarazione sicura di s’ che la CFS/ME sia una
patologia psichiatrica sia non solo arrogante, ma incosciente. Dovreste
imparare dal vostro stesso passato.
In conclusione, vi
esortiamo a non includere la CFS/ME nel DSM-5, dal momento che non ’
necessario, ’ dannoso e non ’ scientificamente sensato, sulla base
delle conoscenze mediche che abbiamo.
